Q. Do I need a referral to be accepted as a patient?
A. We prefer that our patients be referred by their healthcare providers,However,we also accept self referred patients as long as there is no insurance restrictions.Patients will need to bring all pertinent medical records with them in their initial visit.
Q. What do I need to bring with me on my initial visit?
A. Please bring the following to our office in your consultation day:

Your completed "New Patient Information forms".We sent these forms to you by mail, the day you were scheduled. If you did not receive them, you can download and print these forms by clicking on "FORMS" then "New Patient Packet (including all consents)". If you would like to fill these forms in our office, please arrive at least 30 minutes before your scheduled appointment to be able to complete them before your visit.

Driver’s license or other photo identification

Insurance card or Workman’s Compensation/Claim information

Medical records that you would like us to review.

Imaging CDs (e.g. MRI, CT, X-Ray). If you do not have copies of these images, you will need to go to the radiology facility where you had these images done and request obtaining the "disc or CD of the images". They usually hand you the CD after short time.Please, bring CD with you to our office. Your referring physician may send us reports of the studies but not the actual images.
